Review of the article on “Hairline Correction by Hair Transplantation for Reducing Apparent Face Length in Long-Face Women”
Presented at the ABCRC Paper Club meeting in April 2023.

PRESENTATION
By Dr. Maira Merlotto

Paper Club is a bimonthly meeting to discuss current and relevant scientific articles in our clinical and surgical practice. In the April 11, 2023 edition, the article “Hairline Correction by Hair Transplantation for Reducing Apparent Face Length in Long-Face Women” was presented, whose author Jae Hyun Park is a reference in the subject of female hair transplantation. With several articles published, the author guides objective parameters to assist in the study of the face and surgical planning directed according to the individualities of each patient. The article was presented and discussed by Matheus Pantoja and moderated by colleague Christine Graff. A summary of the work done by the presenter follows.

Regardless of ethnicity, the long face can create aesthetic problems, depending on the following factors: vertical maxillary hyperplasia, narrow face, and lack of mentocervical definition. The vertical proportion of the face in the frontal plane is divided into upper, middle, and lower thirds.

However, the authors point out that the perception of a long face does not only occur in the frontal perspective, but also in the oblique and lateral perspective. Just by filling the frontotemporal recess with hair transplant, a visual shortening of the face is achieved. Therefore, the height of the face can vary by different standards.

Excellent results were achieved by the authors of the article by proportionally reducing the oblique and lateral measurements, without interfering with the frontal measurements.

Cases of 83 female patients, between 23 and 47 years old, were studied. The desired density was 50-60 follicular units (FU)/cm², 35-55 FU/cm² in the frontotemporal recess, and 15-25 FU/cm² in the infratemporal and sideburn areas. The FUE and FUT techniques were used to extract the UFs.

Both the surgeon and the patients had their satisfaction measured using the Likert Scale from 1 to 5, as follows: 1, very dissatisfied; 2, little dissatisfied; 3, neutral; 4, little satisfied; 5, very satisfied. The average satisfaction of the patients was 4.5 and the surgeon’s satisfaction was 4.9. No adverse effects such as asymmetry or infection were reported.

The authors point out that most previous studies have defined or recognized a long face by absolute linear measurement from the frontal hairline to the tip of the chin in frontal view, but long face morphology may be accentuated in those with the two-thirds elongated bottoms. Additionally, female patients with an M-shaped frontotemporal recess may consider hairline correction surgery to improve the age and masculine appearance imparted by this hairline shape.

This study was carried out in East Asia; therefore, all participants were from this origin. Surgical marking and indication may differ in Western patients.

For all these aspects, it is concluded that the length of the face will be better evaluated if its oblique and lateral perspectives are considered, in addition to the absolute frontal view. Global facial proportions must be considered for facial harmony, and making synchronized adjustments to the vertical proportions of the side areas of the hairline helps to aesthetically harmonize the face in individuals with a long face.
